Challenge
Referrals and follow-up lived in inboxes. The EHR held the record and none of the coordination. Patients called because nobody could say what was next.
Approach
Map the handoff. Design a narrow coordination object — not another EHR. Permissions by role. A patient view that only answers “what should I do now?”
Solution
A sample product: a shared task model for the clinic, integrations that write back the minimum, and a calm patient app for appointments and instructions.
